In het eerste deel van The Octopus werd de 13-jarige dochter van politie-inspecteur Corrado Cattani (Michele Placido) door de Maffia ontvoerd en verkracht.
Daardoor is politie-inspecteur Corrado Cattani genoodzaakt zijn strijd tegen de georganiseerde misdaad te staken.
Maar wanneer zijn dochter alsnog komt te overlijden, houdt Cattani de maffia voor haar dood verantwoordelijk.
Wraak is dan ook zijn belangrijkste motief wanneer hij opnieuw de strijd aanbindt met de Italiaanse maffia.
